Objective£ºBased on our experience, we discuss the application and usefulness of plugging method for sparing critical structures and optimization of treatment plan during gamma knife radiosurgery(GKS).
Materials and Methods: We retrospectively reviewed 17 cases using plugging method during GKS from July 2004 to September 2007.
Results: Diagnoses based on magnetic resonance image were vestibular schwannoma in 5 cases, meningioma in 4, trigeminal neuralgia in 4, pituitary adenoma in 2 and others(metastasis, chordoma and giant cell tumor in each one case). The location of lesions were cerebellopontine angle in 9 cases, skull base in 5 and parasellar region in 3. Involved critical structures were cochlea in 7 cases, brain stem in 6 and optic apparatus in 5. The plugging pattern and number were widely diverse and partly depended on the laterality of the lesion and the distance between the lesion and critical structure. The critical structures could be spared within acceptable dose in all cases.
Conclusion: The use of plugging method during GKS guarantees the enough radiation dose for lesion site through
reducing the radiation exposure of the critical structures.
